We’re pleased you’ve decided to pursue treatment from Premier Prosthodontics and we’re certain you’ll soon be among our many satisfied patients.

To expedite your first office visit, and for your convenience, you may complete new patient forms in advance. We can have all necessary forms sent via text, e-mail, or by mail. If you are unable to complete the forms before your visit, please allow at least 15 minutes for completion in the office before you see the doctor.
